The conditions in which people live, work, learn and play affect their well-being. Addressing food insecurity, housing and other factors is key to closing disparities in health outcomes.
Every patient receives a social services screening to identify their needs. Our FQHCs coordinate within the PHMC family of programs and with other community-based social service providers to connect patients to the resources below:
- Health insurance coverage
- Supplemental Nutrition Insurance Assistance Program (SNAP) benefits
- Utility payment assistance
- Legal assistance services
- Access to temporary or permanent housing
