Mr. B.

PHMC Health Connection Patient

Mr. B. visited PHMC Health Connection in late 2019 and raised areas of minor discomfort. Upon further evaluation, his medical provider learned that he suffered from severe shoulder back pain, hypertension, and severe bilateral blurry vision. He also lacked health insurance and was reluctant to speak with a Behavioral Health Counselor despite experiencing anxiety and depression.

PHMC Health Connection's team continued to follow up regularly with him after his initial appointment. Over time, Mr. B. began to share more about his life and health situation. This allowed staff began to collaborate to address his various needs. As he grew more comfortable, the health center's medical providers and the referral coordinator connected him to various specialists, including an on-site trauma therapist. The social worker also helped him get food stamps, health insurance and SSI. Today, Mr. B remains a loyal, healthy patient of PHMC Health Connection.

PHMC Care Clinic Patient

A long-term patient at the PHMC Care Clinic had been receiving treatment and case management support to address his opioid use disorder. Nearly two years ago, he began to struggle with alcohol dependence, experienced more negative thinking, and lost access to housing. The health center staff connected him to the PHMC CHANCES program so he could receive outpatient treatment. Additionally, he worked with his case manager to obtain housing through Project HOME and to enroll in job training. Eventually, he completed basic computer coursework and training to become a peer recovery specialist. The patient is grateful for the support of the PHMC Care Clinic in changing his negative thinking, instilling hope and helping him stay on track.
