Locations PHMC Health Connection

PHMC Health Connection provides primary care services to children, teens and adults. Our health center partners with Norris Homes and Apartments, which are owned and operated by the Philadelphia Housing Authority. Our team includes bilingual staff and offers interpretive services. PHMC Health Connection is a Patient-Centered Medical Home.

Our health care services include:

• Primary care for adults, children and families
• Vaccinations
• Family planning, including contraceptive counseling (IUD and Nexplanon placement) and education
• Women’s health services
• Behavioral health evaluations and consultations
• Testing, counseling, and treatment for HIV and Hepatitis C
• Medication-Assisted Treatment and peer recovery services
• Health and nutrition counseling
• Management of chronic health conditions (diabetes, hypertension, asthma, etc.)
• Medical case management and referral navigation
• Social services assistance (SNAP, housing, insurance, utilities, legal, etc.)

Most insurance plans are accepted. A sliding fee scale is available for uninsured and underinsured patients. We see all patients regardless of their ability to pay.

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). This health center also receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
